Care Manager

Care Manager (Remote) Location: Remote (EST hours) Employment Type: Indefinite Contract Start: 10/26 - no days off within first two weeks Schedule: Monday–Friday, 9:00 AM – 5:30 PM EST 40 hours/week (Includes two paid 15-minute breaks and one unpaid 30-minute lunch) Equipment: Must have your own laptop (cannot be Chromebook or Macbook) Pay Rate based on Care Management Experience: 1 year experience: $21/hr 2 years experience: $22/hr 3 years experience: $23 4+ years experience: $24/hr The Care Manager supports care coordination efforts for New York Medicaid members by managing referrals, follow-ups, and member engagement. This role focuses on improving health outcomes through proactive outreach, documentation, and collaboration with providers and community partners. Step 1: Cold Screening Step 2: Eligibility Assessment Step 3: Service Connection Step 4: Service Delivery Step 5: Follow-Up and Monitoring Required Skills & Qualifications High School Diploma or GED Care management experience Experience with Medicaid and Medicaid patient populations Reliable high-speed internet with Ethernet connection (required) Comfortable with inbound and outbound calling with strong communication and customer service skills Tech-savvy and able to navigate multiple systems Ability to work independently in a quiet, distraction-free workspace
