Job Description
Job Title: Licensed Vocational Nurse (LVN)

Profession: Home Health

Experience: Preferred experience in home health care

License: Licensed Vocational Nurse (LVN) licensure in the state of practice required

Certifications: Basic Life Support (BLS) certification required; Arrhythmia certification preferred

Must-Have: Knowledge of HCHB and EPIC required

Description:
Our home health and hospice agencies serve the entire community and offer personalized in-home care.
Job Summary: Performs procedures that require specialized training for general nursing care and related services for assigned patients under the direction of a Registered Nurse (RN) or Medical Doctor (MD).
Responsibilities include participating in the planning for and provision of patient care.
Knowledge of asepsis is utilized in the performance of treatments such as sterile dressings, irritations, and cauterizations.
Provides functional direction to ancillary personnel.
On-call duties will be assigned based on operational needs.
If assigned, the incumbent is required to perform on-call duties.


Job Requirements:
Education and Work Experience: High School Education/GED or equivalent preferred; Bachelor's Degree or equivalent combination of education/related experience preferred.


Essential Functions:
Provides care for assigned patients under the supervision of an Registered Nurse (RN) according to established standards and nursing processes.
Performs routine visits based on departmental specific policies and procedures.
Assists in the development of a prioritized interdisciplinary plan of care for each patient.
Implements and coordinates the plan of care utilizing appropriate organizational skills and reporting mechanisms.
Implements individual teaching plans, including Patient Teaching Protocols and discharge Plans based on mutual goal setting with an emphasis on patient decision-making.
Performs assigned clinical procedures (i.e. phlebotomy, injections, drug screens, etc.).
Administers medications.
Orders and restocks supplies as assigned according to established par levels.
Maintains neatness and cleanliness in assigned areas.
Performs other job-related duties as assigned.
