Key Responsibilities\n

• Report to the Social Work Manager and actively participate in evaluating, diagnosing, and treating a wide range of psychosocial struggles faced by patients and their families.

• Apply clinical social work skills to assist patients in overcoming personal and environmental challenges that hinder their psychosocial functioning due to mental or physical conditions.

• Ensure adherence to social work professional standards, maintaining compliance with the NASW Code of Ethics.
\n
• Conduct comprehensive bio-psychosocial evaluations aimed at enhancing patient health status and ensuring smoother care transitions.
\n
• Collaborate with medical and support staff to develop comprehensive care programs that enhance treatment compliance and facilitate effective care coordination.

• Create individualized treatment plans, utilizing both individual and group therapeutic modalities, while regularly analyzing treatment outcomes to optimize care.

• Document all social work activities consistently in patients' medical records per departmental standards.
\n
• Educate patients and their families on medical and social work recommendations to support the restoration of optimal functioning.

• Assess advocacy needs and coordinate community resources to empower patients, promoting self-determination and enhanced social support.

• Maintain statistics and reports in compliance with departmental and industry standards, while staying updated on available community resources.

• Follow all departmental policies and procedures to ensure a safe and compliant environment.

Patient Population / Client Focus\n

You will primarily work with individuals in a detox setting, supporting those struggling with substance use disorders. This role requires a deep understanding of addiction, recovery principles, and the complexities of mental health issues. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution, accompanied by a current license from the State Board of Social Work Licensure.

Compensation and Benefits\n
