Hospice RN Case Manager

Position Overview Seeking a Hospice - Registered Nurse Case Manager to provide compassionate end-of-life care in patients' homes or facilities. The role involves assessing patient needs, developing and implementing personalized care plans, and collaborating with a multidisciplinary team to ensure quality, patient-centered care. The nurse will evaluate changes in patient conditions, communicate effectively with families and team members, and advocate for culturally sensitive support. This safety-sensitive position requires a valid RN license, a valid driver’s license, and the ability to work in a safe, alert manner.
