Description

• Conduct thorough evaluations of patients in either inpatient or outpatient environments to create tailored treatment plans
• Implement therapeutic interventions focused on enhancing mobility, strength, balance, and overall physical function
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes
• Provide comprehensive education to patients and their families regarding treatment objectives, discharge planning, and at-home exercises
• Maintain accurate records of assessments, interventions, and patient progress in compliance with established protocols
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive treatment approaches

Education

• Doctorate in Physical Therapy (DPT) or a related accredited degree

Certifications

• Current state licensure as a Physical Therapist
• Preferred CPR certification
• Additional specialized certifications (e.g., geriatric, neurological) are an added advantage
