Home-Based Palliative Care Nurse Case Manager

Hospice of Humboldt Jobs in Eureka, CA is seeking a Palliative Care RN (Case Manager) for the day shift to join a compassionate interdisciplinary team delivering home-based care to patients with serious illnesses.
You will coordinate care, educate families, manage symptoms, and develop care plans in collaboration with physicians and local resources, while maintaining a patient-centered approach and ensuring quality of life.
