The Role We are looking for a team-first Physical Therapist who is energized by working alongside a multidisciplinary clinical team. You will carry an outpatient caseload, co-develop integrated care plans with colleagues across disciplines, and contribute to a culture where every provider's voice matters. Responsibilities * Evaluate and treat a diverse outpatient caseload using evidence-based techniques * Collaborate with our multidisciplinary team on shared care plans * Participate in regular team case conferences and clinical huddles * Educate patients on movement, self-care, and lifestyle strategies * Maintain timely documentation in our EHR system Qualifications Required: * Doctorate in Physical Therapy (DPT) from an accredited program * Current or eligible Illinois PT licensure * Strong collaboration, professionalism, and communication skills * Genuine interest in integrative and whole-person care Preferred: * 1+ years of outpatient or wellness setting experience (student clinic or professional environment) * Pelvic floor therapy knowledge or certification (a strong plus) * Familiarity with complementary and integrative medicine (can train) Benefits (Effective After 6 Months) * $65 per patient visit — top-of-market, performance-rewarding compensation * Complimentary in-house wellness treatments * Flexible scheduling Company Description Elevis Wellness Collective is a Chicago-based integrative health practice where physical therapists, acupuncturists, chiropractors, massage therapists, and naturopathic physicians collaborate under one roof.
