Licensed Medical Provider: Nurse Practitioner (NP) or Physician's Associate (PA)

The Licensed Medical Provider at Central City Concern may provide primary care services appropriate to the needs of the population we serve in a variety of different settings. The role will understand and be comfortable with treatment of physical health, mental health, and substance use disorder diagnoses in the context of care to a homeless and underserved population. Primary care at Central City Concern includes low barrier prescribing of medications for opioid use disorder, a harm reduction approach to many health conditions, and a flexible and responsive approach to highly vulnerable patients. Blackburn Center makes it easy to access many kinds of help to gain stability. Our east side location brings Central City Concern's signature health care, housing and employment services together, all under the same roof. You'll find treatment for routine health concerns/primary care, mental health support, care for addiction and substance use disorders, an on-site pharmacy, resources to help you find meaningful work, and housing for more than 100 residents, most of whom are in recovery. Address: Blackburn Center (12121 E. Burnside St. Portland, OR 97216) Schedule: Monday - Friday (Saturdays and Sundays off), 8:00am - 5:00pm Compensation: $135,000 - $176,000, annually. Final salary will be determined based on relevant experience, education, credentials/licensure, and internal equity.
MINIMUM QUALIFICATIONS: ALL APPLICANTS
Demonstrated skills in primary care medicine experience preferred. Must meet CCC privileging requirements as required by FTCA. Must have full prescriptive powers in Oregon with a DEA number. Must have current BLS certification. Wound care experience highly preferred. Spanish language fluency preferred. Valid driver's license preferred. If required to drive, must pass a DMV background check, and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. If required to drive, must pass an initial driver's training within 60 days of being an approved driver and continued recertification training. Graduate of an accredited Nurse Practitioner Program with the appropriate scope of training and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure. Appropriate board certification.MINIMUM QUALIFICATIONS: PHYSICIAN ASSOCIATE
Graduate of an accredited Physician Associate Program and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure. Board certification with NCCPA.ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential Clinical and Compliance Duties Work as a member of the primary care team to deliver comprehensive, patient‑centered services. Take appropriate health histories, perform physical exams, review vitals, labs, other studies, prescribe medications, perform procedures, review outside medical records and other forms of treatment as necessary and appropriate to the standard of care being provided in the setting for both acute and chronic health concerns. Triage, diagnose, manage minor and/or acute illnesses and initiate treatment. Along with the care team, support patients in achieving health resiliency and engagement by utilizing techniques such as shared decision making, motivational interviewing, continuity relationship building, health education, and preventive care appropriate to patient characteristics. Timely completion of appropriate medical records. Attend all mandatory CCC training in a timely manner. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This position will provide after‑hours phone coverage on scheduled rotation with colleagues. Calls will be addressed first by contracted nurse triage service, LMPs will be the second call. Collaborative Care Support pharmacy‑based collaborative drug therapy management programs and provide medical visits to support clients receiving treatment. Coordinate with other CCC programs supporting patients in a continuum of services such as Hepatitis C treatment, withdrawal management, residential treatment for substance use disorders, specialty mental health, and overdose prevention programs Quality Improvement and Practice‑Based Learning Identify opportunities for improvement in medical workflows and logistics. Participate in review and collection of performance and quality Indicators; review and contribute to continuous improvement opportunities and plans. Participate in and, at times, lead provider education sessions on a variety of clinical topics, including helping train and onboard other medical providers and staff. Participate in compliance, quality assurance, and quality improvement activities as directed. Collaborate with the care team to ensure optimal performance on team‑based performance metrics. Perform other duties as assigned, including work at other CCC clinical sites and/or off‑site at community events.SKILLS AND ABILITIES
Individuals must have the ability to work in a multi‑disciplinary, team‑oriented environment. Must be able to interact with co‑workers, patients and staff to instill confidence and facilitate communication. Experience in primary care or mobile medical practice. Ability to consider the impacts and outcomes for underserved communities during decision‑making processes with attention to trauma informed principles, accounting for issues relating to social equity. Ability to display a non‑judgmental attitude. Sufficient manual dexterity and physical ability to perform assigned tasks with reasonable accommodation as needed. Ability to manage time and meet deadlines. Knowledge of de‑escalation methods or willingness to be trained in these methods. Comfort working in an independent clinical practice with on and off‑site support from medical and administrative supervisors.
