About the Opportunity / A Day in the Life:

This is a role for an FNP or AGNP who wants meaningful patient relationships, clinical autonomy, and the ability to take ownership of a practice. You will provide primary care to older adults living in dementia care, assisted living, and independent living communities across Lake Havasu City.

Your day centers on the full spectrum of geriatric primary care — chronic disease management, preventive care, acute concerns, medication review, and follow-up visits. Many patients are medically complex, so the work rewards strong clinical judgment, patience, and the ability to balance multiple conditions while keeping quality of life and independence at the center of every plan.

You will practice with a high level of independence while keeping physician support within reach. You will also work closely with facility staff, nurses, therapists, specialists, patients, and families to coordinate care and catch changes in a patient’s condition before they become larger problems.

Job Duties & Responsibilities:

• Provide comprehensive primary care to residents of dementia care, assisted living, and independent living communities
• Evaluate and manage chronic disease, acute medical concerns, and medically complex geriatric conditions
• Conduct preventive care, routine assessments, and follow-up visits
• Develop individualized treatment plans focused on health, function, independence, and quality of life
• Manage medications and identify opportunities to reduce unnecessary polypharmacy when clinically appropriate
• Monitor changes in patient condition and adjust treatment plans accordingly
• Collaborate with physicians, nurses, therapists, specialists, facility staff, patients, and families
• Coordinate patient care across facilities, specialists, hospitals, and other clinical settings
• Educate patients and families on diagnoses, medications, treatment plans, and goals of care
• Maintain accurate, complete, and timely documentation in the electronic medical record
• Exercise independent clinical judgment and seek physician collaboration when appropriate

Required Qualifications:

• Licensed Nurse Practitioner with FNP or AGNP preparation and eligibility to practice in AZ
• DEA registration, or the ability to obtain one
• Strong clinical decision-making and critical-thinking skills
• Excellent interpersonal and communication skills
• Ability to practice both independently and collaboratively
• Reliable transportation for local travel between facilities

Preferred Background:

• Two or more years of experience caring for geriatric patients
• Experience in assisted living, dementia care, long-term care, internal medicine, family practice, hospice, or palliative care
• Experience managing medically complex older adults
• Familiarity with medication management and care coordination in senior living settings
