Licensed Outpatient Therapist

Outpatient Therapist – Bachelor's/Master's Level (Mental Health Worker)

This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload. This position also ensures completion of required clinical documentation related to individual caseloads in a timely manner.

Essential duties and responsibilities include:

• Demonstrates proficiency with assessment skills by completing psychosocial assessments at in-take and in on-going intervals as outlined by program requirements; completing standardized assessments and/or outcome-based measures.
• Developments individualized treatment plans by adhering to applicable county, state and federal standard regulations when developing individualized treatment plans for consumers.
• Provision of therapy to consumers in individually recommended setting by adhering to recommendations outlined in individual consumer treatment plans.
• Provision of treatment of the whole person by providing service in collaboration with all team members involved with the consumer, and with the understanding that treatment of the whole person is crucial.
• Evaluates individualized consumer-drive treatment plan reviews by completing on-going evaluation of efficacy of treatment evaluations, via development of treatment plan reviews.
• Develops transition and discharge planning by evaluating recovery and progress through treatment goals, symptomology.
• Completes all required clinical documentation by completing progress notes per service, including necessary signatures, discharge summaries, incident reports, treatment reviews via electronic health record system.
• Responds to emergency/crisis situations with team members and consumers by committing to ensuring the safety of the consumer, and the proper utilization of de-escalation skills, when needed.
• Maintains assigned, planned schedule by following program guidelines to ensure regular service provision and maintenance of caseload, per agreement with program supervisor.
• Manages and supports coordination of care through routine conversations and referrals when needed (i.e. natural supports identified by the consumer, outside referrals, PCPs, school districts, etc.).
• Maintains a commitment to professional development by participating in on-going supervision, training, and department and team meetings, as outlined by policies and procedures.
• Adheres to professional and ethical interaction policies and procedures by interacting professionally, ethically, and respectfully with consumers, other team members – including family members, and coworkers, at all times; following policy and procedure and maintaining appropriate professional boundaries.
• Supports on-going and future business needs by completing other duties, as assigned, to effectively support business priorities and processes.

Physical demands may include remaining in a stationary position 50% of the time, occasionally moving about inside a home, school, community, clinic, or program office environment to provide services to consumers, support employees and team members, or access office resources and machinery. May be required to operate a computer and other office productivity machinery (i.e.: iPad, copier, printer). May be required to occasionally ascend/descend stairs to provide services to consumers, support employees and team members, or access office resources and machinery. May be required to position self in appropriate physical proximity to provide services to consumer, support employees and team members, or access office resources and machinery. May be required to frequently communicate with consumers, employees and team members, or external sources in a way that supports the company mission and enhances delivery and quality of services. May be required to recognize and determine data and information pertinent to essential job functions. May be required to frequently transport resources and materials required to provide quality services. May be exposed to a variety of work settings, including home, school, community, clinic, and program office environments.

Work environment may include home, school, community setting, or outpatient clinic – as outlined in individual consumer treatment plan. Up to 100% of travel between appointment sites or other locations – as outlined in program requirements.

Required education and/or experience includes a Bachelor's degree from an accredited college or university in Psychology, Social Work, nursing, rehabilitation or activity program. Or has a graduate degree in a clinical discipline with 12 graduate level credits in mental health or counseling. Preferred education and experience includes at least 1 year of experience with either a CASSP service system, or employed by a licensed mental health services agency, or subcontracted agency, or other mental health clinicians with documents training in the field of behavioral modification techniques.
