Hospice RN & Case Manager: Lead with Heart in Home Care

Heartland Hospice

Heartland Hospice is seeking an experienced Registered Nurse to serve as an RN Case Manager, providing skilled nursing care to patients in their homes or other settings in Michigan. You will coordinate with an interdisciplinary team to create and update personalized care plans, educate families, and support end-of-life care with compassion.
