Intake & Transition Navigator for Housing & Care
ACMH
ACMH is seeking an Intake and Transition Specialist in Queens (Kew Gardens) to support admissions and transitions to independent living. The role reports to the Director, Intake & Transition Services and operates on a Monday–Friday schedule from 9:00am to 5:00pm.
The position requires an MA in psychology or related field (or BA with substantial experience in mental health services) and involves coordinating screenings, referrals, and housing transitions, while contributing to ongoing client
