Compassionate Hospice RN Case Manager

Gentiva Heartland Hospice is seeking an experienced Hospice RN / RN Case Manager to lead and coordinate patient care in home settings. You will assess, plan, and deliver skilled nursing services, collaborating with an interdisciplinary team to support patients and their families through end-of-life care. Strong clinical judgment, compassion, and excellent communication are essential.
