Care Manager

Location: Longmeadow, MA, US Reports To: AVP for Health & Wellness Schedule: Mon-Fri 9am-5pm, On-call as needed Salary Range: $52,000.00 to $58,000.00 Annually Position Summary The Care Manager provides casework, care coordination, and support services to Glenmeadow residents, members, clients, and their families, as well as clients in the greater Western Massachusetts region. This role supports individuals in navigating aging-related changes, accessing appropriate resources, coordinating services, and maintaining independence, dignity, and quality of life. The Care Manager works collaboratively with families, Glenmeadow teams, medical providers, community agencies, and external partners to support person-centered planning, service coordination, and continuity of care. Care Management & Service Planning Assess and evaluate individual needs and develop, implement, and update care management or service plans. Review service plansand performfollow-up todeterminethe quantity, quality, and effectiveness of services provided. Assist residents, members, and clients in identifyingneeds, resolving concerns, and making effective use of available resources. Schedule and offer transportation to medical appointments, with support and advocacy during the appointments. Help individuals and families understand the aging process and how it may affect health, lifestyle, independence, and support needs. Support individuals in adjusting to aging-related changes in ways that promote comfort, independence, dignity, and self-esteem. Encourage individuals to participatein decisions and complete tasks independently whenever appropriate to preserve autonomy and confidence. Resident, Member, Client & Family Support Provide supportive counseling, guidance, and resource coordination to residents, members, clients, and families. Work closely with families to provide appropriate support, including individual consultation, family meetings, caregiver guidance, and connection to community resources. Serve as a liaison between individuals, families, Glenmeadow departments, community agencies, medical providers, and other service partners. Provide consultation to staff regarding specific cases, service needs, or concerns involving residents, members, or clients. Collaboration & Community Coordination Work collaboratively with medical, nursing, wellness, Glenmeadow at Home, and other interdisciplinary team members to support coordinated, person-centered services. Develop and maintain relationships with community agencies, health care providers, local service organizations, and other partners to support access to specialized services. Coordinate referrals and follow-up services as needed to support continuity of care. Support resident admission review processes as requested. Documentation, Administration & Program Support Prepare and maintain accurate written case records, reports, service documentation, forms, follow-up notes, and case closing documentation. Complete administrative tasks required to support care management services and departmental operations. Identify opportunities to develop or enhance programs and services that support resident, member, client, and family needs. Perform other duties as assigned. Knowledge, Skills, and Abilities Strong knowledge of aging-related needs, psychosocial supports, community resources, and care coordination practices. Ability to assess needs, develop service plans, monitor progress, and adjust supports based on changing circumstances. Strong interpersonal and communication skills, with the ability to work effectively with older adults, families, staff, providers, and community partners. Ability to maintain confidentiality, professional boundaries, and accurate documentation. Strong problem-solving, judgment, organization, and follow-through skills. Ability to manage competing priorities and respond appropriately to changing or sensitive situations. Ability to work collaboratively across departments and contribute to a supportive, respectful team environment. Comfort using technology and documentation systems to support communication, service planning, and reporting. Commitment to resident-centered service, dignity, independence, safety, and quality improvement. Qualifications Associate or bachelor's degree in social work or a closely related field required. Minimum of three years of experience working with older adults or the senior population preferred. Master’s degree in social work or a related field may be considered with at least one year of related experience. Experience in care management, case management, counseling, gerontology, senior services, health care, assisted living, home care, or a related setting preferred. Certificates, Licenses, and Registrations Care management certification preferred, if applicable. Valid state-issued driver’s license preferred, if travel or transportation support is required.
