Job Requisition: SOCIA001578 Employment Type: Full-time Shift: Days Hours Per Week: 40-hour/week Hiring range: $25.00-$32.00 hourly The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities:

• Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
• Initiating appropriate interventions for residents experiencing mental or behavioral health issues
• Identifying individual social and emotional needs
• Participate in the planning of the resident's admission, community re-entry, or transfer to another facility
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
• Other duties as required for the safety and care of the residents

Qualifications:

• Nursing Home or Hospital experience. Internship is sufficient. (Required)
• BSW, MSW or BA/BS in related field. (Required)
• Bi-lingual English/Spanish (preferred but not required)

Education:

• Bachelor's Degree required
• Minimum years of experience - 1 year
