Job Summary As a member of a multidisciplinary health care team, medical social workers provide psychosocial interventions to patients and families in any department of the LPCHS. Work typically addresses a variety of biopsychosocial issues, includes assessment of the social needs related to the patient’s health status, and development of a treatment plan. Responsibilities also include advocacy within the organization and community, counseling, referrals, practical assistance, consultation, orientation and training to health care staff on the effects of biopsychosocial, cultural and economic issues, and advocacy for social work core values and ethics within Stanford Medicine and the larger community. Licensed Clinical Social Worker Relief LCSW is the second, fully‑qualified and fully‑proficient level of professional social work for employees licensed by the State of California as LCSW. Positions at this level are characterized by substantial professional responsibility and independence of action for providing social work services. Social Work Clinician Social Work Clinician is the entry, developmental and first working level of professional social work for employees who are recent graduates or who have professional social work experience but are not yet licensed as LCSW by the State of California. Essential Functions – Licensed Clinical Social Worker Provide individual and group supervision to MSWs and student interns, as assigned. Prepare and maintain electronic medical records in accordance with department and/or health system policies. Provide written and oral reports and hand‑offs. Conduct psychosocial assessment of patients/families upon admission, annually, or as clinically indicated to evaluate social, emotional and physical needs by applying social work theories and techniques, and identify and address barriers to care. Collaborate with interdisciplinary health care teams to devise, develop, and implement patient/family psychosocial treatment plans, coordinate referrals to specialty and social services including entitlement programs, and record progress notes and continuity of care. Help patients/families cope with environmental and psychological issues of illness. Manage independently assigned caseload of varying clinical acuity. Engage in case finding for patients/families with potential psychosocial needs; attend team conferences and participate in interdisciplinary rounds as needed. Provide supportive counseling, grief counseling, crisis intervention, and other therapeutic services to assist patients and families to face challenges such as coping with illness, processing medical information, and changes in the level of functioning and care needs. Provide psychoeducation to patients and families regarding adaptation to the patient’s diagnosis, illness, treatment, and/or prognosis. Assist patients and families in the transition from youth to adult services. Participate in quality assurance and community improvement as they relate to social work services or needs of patients and families and contribute to the review and development of written policies, procedures, or guidelines related to social work services. Qualifications – Minimum Education: Master’s degree in Social Work from a CSWE‑accredited college or university program (or equivalent). Experience: Two (2) years of progressively responsible and directly related work experience. License/Certification: Valid California LCSW license issued by the California Board of Behavioral Sciences. Education (Social Work Clinician): Master’s degree in Social Work (MSW) from a CSWE‑accredited program; or, if not accredited, a certificate from the California Board of Behavioral Sciences verifying registration as an Associate Clinical Social Worker. Knowledge Ability to apply theories, principles and practices of medical social work and social services delivery systems in a hospital or medical center setting. Ability to assess needs of patients and families using a data‑based approach. Ability to manage a caseload, maintain confidentiality, and determine appropriate course of action in emergencies and stressful situations. Ability to provide written case narratives and reports; knowledge of effective assessment, interviewing and counseling techniques; knowledge of social work theories and hospital resources. Knowledge of child development, family dynamics, cultural considerations and the impact of injury and illness on families. Additional Information – Pay Range Compensation is based on the level and requirements of the role. Salary within our ranges may also be determined by education, experience, knowledge, skills, location, and internal equity. Typically, new team members join at the minimum to mid salary range.
