Registered Nurse — Home Health

Schedule: Friday–Monday | Weekend on-call rotation
Productivity: 30 points/week
License: Indiana RN license or Compact license accepted
Mileage: $0.43/mile
Coverage: Clark, Floyd, Scott, Jefferson, Crawford, Harrison, Washington & portions of Orange County, IN

We’re seeking an experienced Home Health RN for a Baylor schedule serving patients throughout Southern Indiana. This role is ideal for an RN with strong clinical skills and experience managing complex home-care needs.

Key Responsibilities

Perform comprehensive patient assessments and develop individualized care plans.
Provide skilled home health services and coordinate care with physicians and other healthcare professionals.
Manage care transitions and connect patients with appropriate medical and community resources.
Monitor patient progress and identify opportunities for prevention and improved outcomes.
Maintain accurate clinical documentation and advocate for patient needs.

Required Skills

Home Health experience required
HCHB experience required
Wound care
IV therapy
Drain care
G-tube care
Patient education and basic health teaching

Qualifications

Associate’s or Bachelor’s degree in Nursing or related field.
Current, unrestricted Indiana RN license or Compact license.
Case Management certification.
Strong clinical assessment, critical-thinking, communication, and care-coordination skills.
