Responsibilities include phone triage and patient education. The nurse will assist with minor procedures and medication refills. They will manage medication prior authorizations and communicate diagnostic results. Transitional care management is also part of the nurse's duties. The position requires the ability to perform vital signs assessments and handle patient teaching. Other skills required include blood glucose monitoring and emergency response measures. Knowledge of insurance verification and medication reconciliation is necessary. The nurse should be able to obtain 12 lead EKGs and perform nebulizer treatments. Experience with referrals, scheduling appointments, and follow-up phone calls is essential. Patient interactions may include ages from neonates to geriatrics. Daily patient visits can reach up to 80 individuals. The nurse must be prepared to work with interdisciplinary support services.
