Work Environment\n

This position is entirely remote, allowing you to provide essential support to clients from the comfort of your own home while maintaining necessary flexibility to meet client needs. Standard working hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend availability required based on pressing client situations. As part of our commitment to continuous care, you may also need to be available for on-call support during urgent situations or after-hours, ensuring that our clients receive timely assistance.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, evaluating emotional, mental, and social factors contributing to their overall well-being.

• Collaborate effectively with clients and their families to devise tailored care plans that align with their individual goals, preferences, and challenges.

• Provide compassionate counseling and emotional support to assist clients in navigating significant life transitions and challenges, including recovery from substance use.

• Advocate for clients’ rights and facilitate access to necessary community resources, ensuring they can engage in support services that promote healthy living.

• Work in synergy with interdisciplinary teams, which may include nurses, addiction specialists, caregivers, and other healthcare providers, to ensure holistic care for clients.

• Perform regular visits to maintain oversight on clients’ well-being, addressing any safety concerns and ensuring suitable living conditions.

• Facilitate effective communication among family members involved in a client’s care, helping to resolve conflicts and misunderstandings that may arise.

• Educate clients and their families about available community resources and programs designed to enhance social interaction and overall quality of life.

• Maintain meticulous and up-to-date documentation of client assessments, care plans, progress notes, and other pertinent interactions, adhering to relevant privacy regulations.

• Stay informed on the latest social work practices, policies, and regulations to ensure compliance with ethical standards and best practices.

Your primary focus will be on seniors, a demographic that often faces various challenges such as mental health issues and social isolation, particularly in a detox setting. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory.

• Current and valid state licensure as a Clinical Social Worker (LCSW) is required.

• A minimum of two years of experience within a social work role, ideally in a community center or geriatric setting in a Detox Center environment.
