Work Environment\n

This position allows for a remote work setting, promoting flexibility and work-life balance. Operating hours are from Monday to Friday, 8:00 AM to 4:30 PM, but some evening and weekend hours may be required based on client needs. The role demands your availability to respond to urgent situations, highlighting the importance of on-call support when necessary. Key Responsibilities\n

• Conduct detailed assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors that may influence their well-being.

• Develop personalized care plans in collaboration with clients and their families, ensuring that individual goals and preferences are prioritized.

• Provide counseling and emotional support to seniors and their families, guiding them through life transitions, such as loss or health changes.

• Advocate for clients’ rights within the community, helping them access essential resources, programs, and services specific to their needs.

• Work closely with an interdisciplinary team—including healthcare professionals, caregivers, and nurses—to coordinate care and enhance service delivery.

• Conduct regular check-ins with clients to monitor their well-being, assess their living conditions, and proactively address any emerging concerns or safety issues.

• Facilitate family meetings to improve communication and resolve conflicts regarding care responsibilities and decisions.

• Educate clients and families about community resources, encouraging engagement in support groups and programs that foster social interaction.

• Maintain accurate electronic health records (EHR), documenting assessments, care plans, interactions, and client progress according to compliance standards.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n

The salary for this role ranges from $60,000 to $100,000 per year, commensurate with experience and credentials.
