Position Overview

The Care Manager Assistant - LPN plays a vital role in coordinating patient care and services. This position emphasizes high-quality care through a collaborative approach that involves monitoring, evaluating, and aligning services with the needs of patients, while adhering to all relevant laws, regulations, and organizational standards.

Education, Experience, and Licensure Requirements
  • Completion of a nursing diploma (either an associate's or bachelor's degree in nursing).
  • Current licensure/registration in the relevant state, with an active LPN license in good standing required throughout employment.
  • Preferred: At least one (1) year of clinical experience in a post-acute care environment.
  • Preferred: Previous experience in case management, utilization review, and discharge planning.
Specific Job Requirements
  • Ability to produce written communication that is clear, concise, and well-structured.
  • Strong organizational skills with the capability to prioritize tasks and manage time effectively.
  • Contribute to the organization's objectives and support strategic initiatives.
  • Possess expert knowledge in the relevant field of practice.
  • Make independent decisions when necessary.
  • Familiarity with the practices, procedures, and regulations governing operations in a post-acute care facility.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including general management duties, patient rights, and safety protocols.
  • Maintain professional relationships with colleagues, vendors, and others.
  • Ensure confidentiality of all proprietary and sensitive information.
  • Understand and adhere to organizational policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
Essential Functions
  • Effectively utilize relevant computer systems for data entry and retrieval.
  • Act as a liaison between external case managers, families, physicians, and community resources.
  • Provide training and education to patients, families, colleagues, and other care providers.
  • Implement standards of practice for care management, ethical performance, and care coordination.
  • Deliver excellent customer service and maintain a positive attitude towards patients.
  • Assist in the safe evacuation of patients when necessary.
  • Demonstrate reliable and consistent attendance.
  • Utilize reasoning skills and sound judgment in various situations.
  • Communicate effectively and collaborate within an interdisciplinary team.
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
  • Read, write, speak, and comprehend the English language proficiently.
