amily Nurse Practitioner (FNP) – Full Time
PathCare Health is seeking a full-time Family Nurse Practitioner (FNP) to join our team at our Brooklyn, NY office .
This is a fully in-person position with competitive compensation. We are looking for a reliable, motivated provider who is comfortable working with patients of all ages and managing a steady patient schedule.
Responsibilities:
Evaluate and treat patients of all ages
Perform patient assessments and physical examinations
Diagnose and manage acute and chronic conditions
Review medications and medical history
Develop appropriate treatment plans and referrals
Complete and lock documentation accurately and promptly
Work closely with our clinical and administrative team
Requirements:
Active New York State Nurse Practitioner license
Family Nurse Practitioner (FNP) certification
Active NPI
Must be enrolled with New York State Medicaid and eligible to bill Medicaid
Strong clinical and documentation skills
Dependable and comfortable working in a fast-paced environment
