Plus a first-year $10,000.00 first year success bonus, paid out quarterly. \n The base compensation range for this role is fixed, with a maximum cap of $91,000.00. We are consistently named a Top Workplace by our employees and genuinely care where you are in your career path. \n \n Our high value rewards package: \n \n \n Up to 24 paid holiday and personal days off in year one \n \n Company funded pension, up to $50,000 credit after 3 years of service \n \n DailyPay: Access your money when you want it! \n \n Industry-leading 360 You ™ benefits program \n \n The option to lease a new Toyota or Ford vehicle at a significantly discounted price below MSRP \n \n Certain benefits may vary based on your employment status. \n \n What you’ll do in this role: \n \n \n Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness(es) \n \n Obtain complete/accurate demographic information and medical history, including diagnosis for care and primary care physician information \n \n Introduce the Agency to the patient/caregiver, explain scope of our services and skilled services requested, and coordinate start-of-care visit \n \n Order requested DME, supplies, and medications related to hospice care \n \n Deliver community education as requested for assisted living facilities, skilled nursing facilities, and hospitals \n \n Provide patient education at bedside regarding hospice, disease progression, and end-of-life care \n \n Attend discharge planning meeting as requested, working with the facility team to provide a safe and successful discharge home \n \n Determine hospice eligibility and appropriate level of care \n \n Communicate with the clinical team on all complex, chronically ill patients, assisting staff and referral sources to prevent re-hospitalizations \n \n \n We are looking for compassionate Transitional Nurse Liaisons with: \n \n \n Associate degree from an accredited School of Nursing \n \n A minimum of two (2) years of experience working as a Registered Nurse including acute care or oncology experience, hospice experience preferred \n \n Experience with technologies such as Microsoft Office Suite, EPIC, and HCHB preferred \n \n Able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety \n \n \n \n Ability to accept different lifestyles, cultures, beliefs, and values \n \n Proficiency with computers \n \n Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required. \n \n \n We are an equal opportunity employer and value diversity at our company. \n \n NOTICE: Successful completion of a drug screen prior to employment is part of our background process, which includes medical and recreational marijuana. \n By supplying your phone number, you agree to receive communication via phone or text. \n By submitting your application, you are confirming that you are legally authorized to work in the United States. \n The low end of the range includes compensation related to salary and first year success bonus.
