Key Responsibilities\n

• Contribute to discharge and aftercare planning for clients, ensuring smooth transitions from care settings.

• Provide therapeutic meal support, helping clients develop a healthy relationship with food and nutrition during therapy sessions.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
