Case Manager

Visits with persons on the caseload and attending team meetings Completing or attend meetings on needs assessment; individual service plan development (CSSP) Assisting with planning for new service development Locating residential, vocational, and other needed services depending on each person's needs Monitoring service delivery and ensuring the health and safety needs of each person are being met. Completion of referrals and crisis intervention as needed Developing waiver budgets Completing case notes Acting as an advocate for people we provide services to Requirements Be a licensed Social Worker (Bachelor of Social Work required for licensure) Or licensed Graduate Social Worker (Master's in Social Work required for licensure) Or have a 4-year degree in Human Services, Psychology or Sociology, or related fields Successful clearance of Department of Human Services background check Successful clearance of Motor Vehicle Background Check and acceptable driving record per Company Policy Core Competencies Demonstrates expertise in needs assessment, individual service plan development, and crisis intervention while advocating for clients' health and safety. Proficient in developing waiver budgets and monitoring service delivery to ensure compliance with standards.
