The Therapy Lead provides discipline‑specific clinical support for therapy services (PT/OT/ST) in home health, working with clinical leaders to ensure care is safe, compliant, effective, and aligned with agency standards. *****The pay grade listed in the posting is based on the Physical Therapist (PT) position. Qualifications Master's Degree in Physical Therapy Master's Degree in Occupational Therapy Master's Degree in Speech Language Pathology Licenses and Certifications Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR) Licensed Occupational Therapist (OT) – Illinois Department of Financial and Professional Regulation (IDFPR) Licensed Speech‑Language Pathologist (SLP) – Illinois Department of Financial and Professional Regulation (IDFPR) Experience Requirement Home health – 3+ years Responsibilities Evaluate the patient’s level of function within regulatory requirements by applying diagnostic and prognostic procedures as directed by the physician or podiatrist. Complete comprehensive OASIS assessment and assess patient eligibility for home health care. Review plan of care and orders with primary physician, read‑back, complete immediately, and transmit within the policy established time frame. Update plan of care as needed to meet patient needs. Guide the patient in the use of therapeutic, creative, and self‑care activities to improve function. Observe, record, and report to the physician or podiatrist the patient’s reaction to treatments and any changes in the patient’s condition. Educate and document discharge planning prior to discharge, including communication with physician, insurance source, outpatient visits, and community resources if applicable. Collaboratively communicate and initiate case conferencing as necessary and document interactions (other disciplines, MD, care coordinator, insurers, etc.). Plan and administer prescribed therapy treatment programs for patients to restore function, relieve pain, and prevent disability, injury, or loss of body part. Perform patient assessment tests, measurements, and evaluations (range of motion, manual muscle tests, gait and functional analyses, body part measurements). Record and evaluate findings to aid in establishing or revising treatment programs. Demonstrate comprehension and adherence to regulatory requirements and home care policies regarding practice and documentation content and timeliness. Complete functional assessments and documentation per regulations. Provide discipline‑specific clinical guidance to therapy staff (PT/OT/SLP) to support safe, effective home health care. Serve as a clinical resource for complex cases, assessment accuracy, care planning, and discharge decision‑making. Promote consistent therapy practice standards across assigned teams or regions. Provide clinical insight to leadership regarding therapy practice trends or development needs. Promote compliance with CMS Conditions of Participation, ACHC standards, and Carle Health policies related to therapy services. Assist with review of therapy documentation for clinical appropriateness, regulatory alignment, and quality indicators. Identify and escalated clinical risk trends that may impact outcomes, safety, or compliance. Assist with aligning therapy practices across sites in collaboration with home health leadership. Compensation and Benefits The compensation range for this position is $39.29 per hour – $67.58 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked, and compensation model.
