Key Responsibilities

• Conduct thorough assessments and evaluations on patients, analyzing historical data and performing specific tests to determine treatment needs.
• Develop and communicate personalized treatment plans to patients and their families, ensuring active involvement in goal-setting and decision-making.
• Implement treatment protocols effectively, using clinical judgment based on each patient's unique presentation and continuously evolving healthcare standards.
• Collaborate with a multidisciplinary team, consulting with other healthcare professionals when needed and recognizing changes in patients' conditions that necessitate adjustments in treatment plans.
• Manage discharge planning effectively, ensuring continuity of care with appropriate follow-up recommendations.
• Maintain timely and accurate documentation of patient encounters, including evaluations, progress notes, and discharge summaries, to fulfill compliance and communication needs.
