Clinical Scheduler

The Clinical Scheduler is responsible for the Skilled Nursing, (Registered Nurse and Licensed Vocational Nurse) patient scheduling for HCP of Texas.

Responsibilities include:

• Scheduling patients for Start of Care (SOC) as they are set to "Admit" in the system.
• Discharging care plans as patients discharge from Home Health.
• Communicating and coordinating with the nurses to make scheduling changes as needed to accommodate patients.
• Keying in the frequency into Kinnser Wellsky as the nurses communicate them by email.
• Communicating with the RN's when a ROC is needed and follow up for timely completion.
• Communicating with the RN's when a supervisory visit is required.
• Ensuring that every nurse is meeting productivity and explaining variances in productivity in bi-monthly utilization meetings with upper management.
• Maintaining the weekend report and assigning the proper on-call nurse for each assigned territory.
• Maintaining the territory map and making corrections/adjustments as needed.
• Scheduling the Home Health Aides as requests are made for them by the RN Case Managers.
• Scheduling the Social Worker as requests are made for them.
• Reporting scheduling issues and concerns.
• Providing excellent customer service to the clinician staff, internal team, sales team and the patients and families that we serve.

This is a Full time M-F 8am to 5pm onsite position in Dallas.
