Penn Presbyterian Medical Center (PPMC) has an outstanding opportunity for a Clinical Pharmacist to join the Ambulatory Pharmacy and work alongside some of the best medical providers in Philadelphia.

Summary:

• This established position at Penn Presbyterian Medical Center shares collaborative responsibility for anticoagulation management as well as clinical services in the Cardiology practice including VAD anticoagulation, REMS compliance, and collaborative practice for Heart Failure patients.

Responsibilities:

• Communicates appropriately with immediate coworkers and all other health care staff: Pharmacists, Nurses, Patients and Students, and listens carefully to others in order to respond appropriately.
• Calculates correctly and accurately amount of medication to be dispensed based on days supply allowed, age, etc.
• Monitors patient profiles, drug utilization, and insurance carrier messages to assure medication compliance, patient specific parameters are considered in medication preparation.
• Participates in Entity and Department wide initiatives for Patient /Employee safety.
• Demonstrates an awareness of patient/ employee safety when carrying out daily responsibilities of their position.
• Complies in all activities with accreditation, legal, regulatory, and safety requirements for specific practice setting(s).
• Participates in the established process for assessing, managing, and reporting ADEs.
• Provides support for other staff members toward their overall effectiveness.
• Trains new hire pharmacists, interns, and technicians.

Operational Staff: - Escalates to embedded office team members when necessary - Prepares and dispenses medications using appropriate techniques to ensure accuracy, optimal outcomes of drug therapies - Keeps active, accurate inventory in work area and appropriately restocks area with returned medications per policy to assure workstations are always organized

Clinical Staff:- Designs, recommends, monitors, evaluates and documents patient and age-specific pharmacotherapy

Minimum Requirements:

Pharmacist licensure in states(s) work is performed Ph.D. (Required) Pharmacy PGY1 Residency Training required PGY2 Residency Training in Ambulatory Care / Pharmacotherapy, etc preferred Active license in the state of practice in Pennsylvania and/or New Jersey to administer injectibles
