The Office of SAMH is statutorily responsible for the planning, evaluation, and implementation of a comprehensive statewide system of care for the prevention, treatment, and recovery of children and adults with serious mental illnesses or substance use disorders. Northeast Florida State Hospital (NEFSH) is a 613-bed, licensed mental health treatment facility that provides person-centered services to adults suffering from severe and persistent mental illness. The hospital, located in Macclenny, was established in 1959 and is the largest state-operated civil treatment facility.Northeast Florida State Hospital Is A Tobacco Free Facility. If you are driven, organized, and thrive in a fast-paced environment, we encourage you to apply.Specific duties and responsibilities include:Transports individuals off grounds to community physicians and other medical facilities in an effort to reduce controllable expenses.Assists in keeping the people we serve calm when transporting to and from appointments in the community.Picks up the people we serve meals if their appointment is during mealtime.Documents behavior of the individual while off grounds, if necessary.As a continuation of ensuring a safe and secure environment, transports individuals to ACLF or mental health facility when discharged back to the community.Picks up the individual from the living area.Checks for all luggage, paperwork, and medication as needed.Upon arrival, assists staff with unloading and check-in process.Before leaving facility, obtains signatures on all paperwork, including Transportation Discharge Checklist.Returns forms to the appropriate office.To ensure a safe and secure environment the employee will operate all vehicles within the posted speed limits on NEFSH campus and the community highways.Will follow assigned schedule in an efficient manner so appointments are timely met.As part of our efforts to increase customer satisfaction, transports the people we serve to their work areas and Recovery Centers as scheduled in a timely manner.Keeps a maintenance check on vehicles to reduce cost of controllable expenses.Keeps the vehicles clean and fueled.Notifies supervisor and the hospital mechanic of any problems with the vehicle.Maintains a neat, accurate and legible Vehicle Usage Record on the vehicles.Delivers legal documents to community lawyer or courthouse.Delivers blood to community labs.Pickup x-rays or reports from community hospitals or Physician's office.Other duties as assigned.Knowledge, Skills and Abilities:Ability for prolonged sitting.Ability to read maps and follow directions.Ability to drive all over state and locate unfamiliar places.Ability to travel by airplane.Requires multiple physical activities: Must be able to bend, twist, turn, kneel, push, stoop and walk extended distances.Must be able to lift 50 pounds or push/pull 100 pounds.Must be able to travel up stairs.Must be able to physically operate all equipment necessary to do job tasks, including using a keyboard.Must be able to demonstrate competency in all areas of training, some of which requires moderate to heavy physical activity such as safe operation of job essential tools and equipment, loading and unloading Wheelchair/Stretcher Vans, climbing, CPR, MANDT and Heimlich training.Must be able to work outside for 8 hours with normal breaks and lunch.Must be able to remain alert for a period of 1-16 hours including normal breaks.Must be able to work 16-hour shifts on occasion.Must be able to work around cleaning chemicals and supplies.Exposure to loud noises. Work in inclement weather.Minimum Qualifications:Must be 18 years of age at time of hire.CDL Class B with a P endorsement is preferredSix months of experience working in a medical, psychiatric, nursing, or childcare setting, customer service experience or in working with developmentally disabled persons; or in working in a correctional facility OR college or vocational/technical training from an accredited institution at the rate of 15 semester, 22 quarter or 360 classroom hours; OR do you possess a current State of Florida Certified Nursing Assistant (C.N.A.) license.Ability to attend a 1 - 2-week mandatory training course that will include completion of a CPR certification.
