Fairview is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinics Oxboro located in Bloomington, Minnesota . Join our thriving multi-specialty practice which has been in the community for over 30 years.

Practice Details:

\n

\n
• 12 provider primary care practice which includes 8 Physicians and 3 Advanced Practice Providers.
\n
• Patient encounters average 15-18 patients per day.
\n
• Opening: 1.0 FTE - 34 patient contact hours per week for full time - flexible schedule.
\n
• Call Frequency: 3 weeks per year.
\n
• As part of Fairview s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
\n
• Fully integrated Electronic Medical Record (EPIC).
\n

\n

Qualifications:

\n

\n
• Graduate of an accredited ACGME/AOA Family Medicine residency training program.
\n
• BC/BE with the ABMS or AOA.
\n
• Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
\n
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
\n
• Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
