Intensive Case Manager

Job Description

The Intensive Case Manager provides mobile, community-based case management services to families experiencing homelessness and housing instability. On a daily basis, this individual conducts assessments, develops individualized service plans, and works closely with participants to identify goals related to housing, healthcare, benefits, employment, and long-term stability. The role requires frequent fieldwork, meeting participants wherever they reside, including shelters, healthcare facilities, community settings, and permanent housing placements.

The ICMS Case Manager coordinates resources and supportive services that address mental health, substance use, physical health, income support, and other psychosocial needs. This person assists clients with housing applications, subsidy programs, benefit enrollment, and connections to community providers while promoting housing retention and self-sufficiency. The role also includes conducting crisis assessments and interventions, collaborating with healthcare and behavioral health providers, maintaining compliance with documentation standards, and utilizing evidence-based practices such as Housing First, Motivational Interviewing, Harm Reduction, and Critical Time Intervention. Success in this position requires strong relationship-building skills, independent problem-solving, and the ability to navigate complex systems on behalf of clients.

Skills and Requirements

Bachelor's degree in Social Work, Psychology, Human Services, Sociology, or a related field

2+ years of experience working with homeless families or other high-needs populations

Experience providing intensive or community-based case management services

Experience conducting client assessments and creating individualized service plans

Knowledge of crisis intervention and risk assessment techniques

Understanding of Housing First, Motivational Interviewing, Harm Reduction, and trauma-informed service models Master's degree in Social Work or a related field

Experience working with medically vulnerable individuals or families

Experience supporting individuals with chronic health conditions, mental health diagnoses, or substance use disorders

Knowledge of public housing programs, rental assistance programs, and housing subsidy resources

Experience with Critical Time Intervention (CTI) models
