Travel RN - Case Manager
Location: Wichita, KS
Assignment Duration: This is a 13-week contract position.
Schedule: Day Shift: Monday to Friday, 8:00 AM - 4:30 PM
Job Description: The Travel RN - Case Manager will be responsible for comprehensive case management services including assessment and discharge planning. The position requires collaboration with a team of healthcare professionals in a fast-paced and dynamic environment.
Responsibilities:
- Conduct case management assessments.
- Develop and implement discharge plans.
- Coordinate care for patients transitioning within the healthcare system.
- Collaborate with physicians, mid-levels, and other staff for optimal patient outcomes.
Required Experience / Certifications / Licensure:
- Minimum of 1-year current acute care case management RN experience.
- Active Kansas RN licensure or licensed social worker.
- Current BLS certification.
- Experience with Meditech and Midas is strongly preferred.
