Homelessness Prevention Case Manager II

Catholic Charities Brooklyn and Queens, Inc. is seeking a Case Manager II to support Homebase Homelessness Prevention clients in the Jamaica, NY area. You will provide assessment, intake, brief and long-term case management, and connect clients to essential services. The role requires a BS in Human Services or related field, 1 year of experience in social services, and the ability to travel across the five boroughs.
