Homeless Prevention Case Manager - Hybrid (1 WFH/wk)

BronxWorks seeks a highly motivated Homeless Prevention Case Manager to join our Homebase team in the Bronx. You will screen and assess client needs, develop service plans, conduct housing court advocacy, and manage a caseload of 40-45 clients. The role emphasizes organization, communication, and collaboration with partners. Responsibilities include intake assessments, eligibility reviews, assisting with program applications, referrals, home visits, data entry, landlord coordination, and #J-18808-Ljbffr BronxWorks
