Key Responsibilities

• Provide physical therapy evaluations and treatments to patients in a home health setting
• Complete and maintain accurate OASIS documentation in accordance with home health requirements
• Develop and implement individualized patient care plans based on assessment findings
• Monitor and document patient progress, adjusting treatment plans as appropriate
• Collaborate with interdisciplinary care team to ensure coordinated patient outcomes

Required Qualifications

• Active Physical Therapist license valid in Virginia (in hand at time of submittal or proof of compact license)
• Minimum 1 year of experience in a home health setting
• Proficiency in OASIS documentation
• BLS certification through the American Heart Association (AHA)
• One supervisory reference required at time of submittal

Preferred Experience / Skills

• Experience with home health-specific electronic medical record systems
• Strong communication and patient education skills in a home-based care environment
