• Holiday rotation with core staff

Requirements

• Active RN License
• BLS Certification
• Minimum 2 years of recent RN experience
• Acute Care experience required
• 2–5 years of recent Palliative Care and/or Hospice experience preferred
• Strong clinical assessment, communication, and critical thinking skills
• Excellent organizational and time management skills
• ICU and/or Oncology experience preferred
• ELNEC certification preferred

Patient Population

• Inpatient Palliative Care patients
• Patients meeting Palliative Care screening criteria throughout the hospital
• Average daily census of 55 patients
• Team manages approximately 15–17 new patients per day

Responsibilities

• Manage daily patient caseload
• Review patient charts and assess patient needs
• Conduct goals-of-care discussions
• Provide education on disease processes
• Discuss Advance Directives and POLST forms
• Facilitate family meetings as needed
• Provide end-of-life support and education
• Attend interdisciplinary rounds
• Complete new patient consultations and follow-up visits

Work Environment

• No direct hands-on patient care required
• Patients located across multiple hospital units and floors
• Collaborative team consisting of Physicians, Nurse Practitioners, and Registered Nurses
• Faith-based healthcare organization
