Registered Nurse (RN) - Hospice

RN Case Manager

Typical schedule is Monday to Friday, 8 AM to 5 PM, with on-call and weekend rotation.

Guaranteed hours: 40. Contract length: 13 weeks.

Current RN license in the state

Valid driver's license and reliable transportation

2 years of recent acute care, home care, or hospice experience preferred

Experience with clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred

Familiarity with HCHB is strongly preferred

Provide skilled nursing and coordinate care to patients and their families in a residential setting.

Observe, assess, evaluate, and plan interventions in relation to patients' physical and psychosocial statuses.

Oversee care provided by Home Health Aides and CNAs; evaluate compliance with the plan of care.

Ensure continuity of care through effective communication and case conferences with interdisciplinary teams.

Provide quality care and appropriate symptom management to promote optimal health and quality of life.

Maintain accurate and complete patient records in accordance with agency standards.

Participate in quality improvement activities and in-service training as assigned.
