RN Care Manager, Kidney Disease - Field-Based Impact

Somatus is seeking a Registered Nurse Care Manager, VBC, to lead high-need CKD/ESKD patient care in partnership with nephrology practices. You will coordinate in-home visits, dialysis clinics, and office-based care, guiding the care team, and removing barriers to optimize outcomes. You will travel to member homes and facilities, educate patients, and develop individualized care plans with the multidisciplinary team.
