Home-Based RN Care Manager for CKD/ESKD
Somatus
Somatus is seeking an RN Care Manager to lead VBC efforts focused on high-need CKD and ESKD patients. You will collaborate with nephrology teams, plan and coordinate in-home visits, and educate patients to improve outcomes. This field-based role requires travel within your region, strong care coordination, and excellent communication with providers and patients. You’ll join a team committed to evidence-based, patient-centered care and must have RN licensure, BLS, and experience in #J-18808-Ljbffr Somatus
