KENOSHA HUMAN DEVELOPMENT SERVICES, INC. 3536 52nd Street Kenosha, WI 53144 Division/Department: Housing Assistance Services Location: Main Office Job Title: KYF Rapid Rehousing Caseworker Reports To: Homeless Services Program Manager Type of Position: Full Time Schedule: 40 hours per week or as demanded by the job Starting Pay: $22.60 per hour Position Summary The caseworker for the KYF Rapid Rehousing Project provides comprehensive supportive services to individuals experiencing homelessness in scattered site living arrangements. Using a person-centered, individualized approach to service delivery, the caseworker is responsible for assessing need, developing a housing stability plan and applying individualized interventions to assist participants achieve their self-defined goals. Responsibilities Participate in the program admission process and establish goals Review the Coordinated Entry Assessment and VI‑SPDAT in Wisconsin Service Point Provide clients with a program orientation at intake meeting Acquire releases of information to gather background information and share with required service providers Obtain third‑party verification of homelessness and may include documentation of a mental health diagnosis from a qualified licensed professional Understand and assist with management of program and system performance measures Participate in the bi‑annual Point‑in‑Time Event Collaborate with Emergency Shelters, Homeless Outreach Workers and other service providers working with homeless individuals and families Provide case management services to program participants on a regular and consistent basis as determined by the individual’s case plan Assess, plan, coordinate, implement and evaluate the overall services delivered to the participant Assist clients in obtaining safe, secure living arrangements by teaching apartment searching skills Assist clients in moving in and out of apartments/houses Help participants learn to live in housing, maintain their housing safely, and get along with fellow tenants and the landlord Create a comprehensive Housing Stability Plan with the participant and provide appropriate interventions Help participants create support systems and participate in the community as they desire Provide individualized budgeting and money management services as needed Teach basic, intermediate and advanced social skills based on need Collaborate with landlords and other community service providers Coordinate opportunities for educational advancement (e.g., GED preparation, post‑secondary training, vocational education) Assist with job preparation and attainment (career counseling, job training, dressing and grooming, job placement and maintenance) Assist participants in accessing mainstream benefits (food stamps, childcare assistance, health insurance) as needed Assist participants in accessing mental health, AODA and medical services as needed Assist with ensuring participants have access to food and clothing Provide transportation and assist in developing transportation skills Develop and run weekly Life Skills Groups Work with the client to establish a discharge plan six months prior to program exit and intensively work to ensure exit to self‑sufficiency and housing stability Ensure documentation is complete and accurate Use Client Tracking System to document client contacts and progress Complete data entry into Wisconsin Service Point (WISP) as required Complete check requests on a monthly basis and submit on time Complete and maintain client files and ensure the information matches the data entered in WISP Complete and file discharge plans appropriately Qualifications Bachelor's degree in a human services related field and/or experience working with individuals and families experiencing homelessness and/or other issues that put individuals at risk of housing instability Effective communication skills, both written and verbal, in English Reliable transportation and a valid driver’s license with a good driving record Own auto insurance with at least 100,000/300,000/100,000 coverage Ability to maintain records and document case notes daily Ability to give and accept feedback as part of a team Ability to traverse the workspace and community frequently Willingness to learn and work with the treatment philosophies Bilingual in Spanish is a plus Kenosha Human Development Services is an Equal Opportunity Employer.
