Key Responsibilities\n

• Conduct in-depth psychosocial assessments utilizing a range of techniques, including interviews, observations, and review of medical histories, to evaluate the emotional, social, and environmental influences on each patient’s recovery.

• Provide compassionate counseling and emotional support to patients and their families, helping them navigate adjustment issues and enhance their emotional well-being during the rehabilitation process.

• Partner with a multidisciplinary rehabilitation team to develop comprehensive, individualized treatment plans, integrating the psychosocial specifications of each patient’s journey.

• Guide patients in accessing vital community resources such as support groups, vocational rehabilitation, and housing assistance to bolster their recovery outside the center.

• Offer ongoing education to patients and their families regarding community resources and post-rehabilitation services, ensuring they are well-equipped for continued support after leaving our care.

• Participate actively in discharge planning, facilitating smooth transitions for patients returning home or moving to alternative care facilities.

Patient Population / Client Focus\n

You will work with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological conditions. Each patient's journey will be unique, and your insights into their emotional and social contexts will be crucial in tailoring effective rehabilitation strategies that bolster their resilience and long-term success.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.

Compensation and Benefits\n
