Case Manager, Welcome HOME Program

Principle Function: The Welcome H.O.M.E. Case Manager builds relationships with young adults ages 18-24, providing support and guidance to individuals who are going through challenges. The Case Manager helps clients develop their skills and confidence and assists them in reaching their personal goals. This position assists the Welcome H.O.M.E. Director in implementing and facilitating a client-driven calendar of events and activities. The Case Manager also receives intake calls, assesses clients' needs for service, and relays information about services provided by the Welcome H.O.M.E. Drop-In Center.
Education and Experience: A bachelor's degree in social work, psychology, or human services is required. Master's degree in social work preferred. Two years of work experience in areas of job responsibilities preferred. Skilled in Trust Based Relational Intervention (TBRI®). First Aid and CPR training, reliable transportation, approved criminal background report.
