This position is primarily remote, allowing for flexibility and convenience for both our clients and staff. However, you will work closely with a dedicated team of mental health professionals at our Hospice Center in Stockton, California, fostering a collaborative approach to patient care. Regular virtual meetings and case discussions will enable you to contribute to a cohesive care model while maintaining essential connections with your colleagues.

Key Responsibilities\n

• Facilitate therapeutic sessions with clients, utilizing evidence-based methodologies to tailor interventions to individual needs.

• Collaborate effectively with healthcare prescribers to align therapeutic and medication management for clients, ensuring comprehensive care delivery.

• Monitor client outcomes actively, participating as a crucial member of the healthcare team to adjust treatment plans as necessary.

• Engage in continuity of care and discharge planning, guiding clients through advance care planning processes to ensure their needs are met beyond their stay.

• Develop and implement individual treatment plans based on thorough psychiatric assessments, focusing on identified issues and specific treatment goals.

• Document all client interactions and care activities accurately in our electronic health record system in compliance with established policies, ensuring transparency and continuity of care.

Patient Population / Client Focus\n

You will primarily work with individuals who are navigating various mental health disorders, providing you with the opportunity to impact lives significantly. Our patient population includes those experiencing anxiety, depression, trauma-related conditions, and more complex needs often associated with hospice care. Your empathetic approach and therapeutic expertise will be pivotal in helping them lead healthier and more fulfilling lives.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related field is required.

• Current California state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
