Work Environment\n

This position is fully remote, operating out of our Detox Center that is equipped to provide a range of services for our target population. You will be part of a dynamic team working collaboratively with other professionals to deliver integrated and holistic care. While most interactions occur via phone or video calls, there may be occasions for in-person visits as required by client needs.

Key Responsibilities\n

• Conduct thorough assessments of seniors’ psychosocial needs by evaluating emotional, mental, and social parameters affecting their lives.

• Develop tailored care plans in collaboration with clients and their families, ensuring that their individual goals and preferences are central to service delivery.

• Provide counseling and emotional support to seniors, helping them navigate life transitions, including health challenges and changes in living situations.

• Act as an advocate for clients, facilitating access to essential community resources and services that promote their overall well-being.

• Collaborate with an interdisciplinary team including healthcare providers, caregivers, and other professionals to coordinate effective care strategies.

• Engage in regular client visits to monitor their well-being, assess living conditions, and address any potential safety issues.

• Educate clients and their families about available community support and resources, enhancing their engagement with local programs that foster connection and social interaction.

• Maintain accurate and up-to-date records, including client assessments, care plans, progress notes, and other critical documentation.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n
