Home Health Physical Therapist Assistant (PTA) – Full Time – Sonoma County Santa Rosa, CA, USA Responsibilities Provides direct physical therapy under the direction of a qualified physical therapist and per the physician’s plan of care, consistent with the state Physical Therapy Practice Act. Treats patients to relieve pain, restore function, and maintain maximum performance. Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training. Utilizes physical agents including heat, cold, water, light, electricity, ultrasound, and massage. Observes and reports patient responses and changes in condition to the therapist and organization personnel. Instructs patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices. Maintains required records, including clinical and conference notes, and integrates them into the patient’s record within prescribed time frames. Participates in home‑care team meetings and inservice education programs. Performs additional duties as assigned. Adheres to company attendance and reliability guidelines to ensure quality and consistency of patient care. Qualifications Graduate of an accredited physical therapy assistant program with an associate’s degree in science or applied science approved by the American Physical Therapy Association. Approved as a physical therapist assistant by the Physical Therapy Board of California. Strong verbal and written communication and organizational skills. Current CPR certification. Valid California driver’s license and reliable automobile in good working order, insured per organization requirements. Physical Requirements Ability to sit occasionally for 1–3 hours. Ability to stand for 3–6 hours. Ability to drive or operate a foot control for 1–3 hours. Ability to walk occasionally on level surface for 1–3 hours. Ability to walk intermittently on uneven surface for less than 1 hour. Ability to bend or climb intermittently for less than 1 hour. Ability to twist, kneel, crouch and crawl for 1–3 hours. Ability to lift, carry, push, and pull up to 50 pounds intermittently in less than 1 hour.
