Job Description
The Diabetes Education Program Coordinator/Instructor supervises the day to day operations of the Diabetes Self-Management Education (DSME) Program. In collaboration with the Executive Director Ambulatory Services, this includes the oversight of budgeting projected visit volume, expenses and revenue, work flow, human resources (staffing) to support the Education program and interfacing with Marketing to promote the program to referring physicians in the primary and secondary service areas of Stamford Health. The incumbent is a Registered Nurse/Certified Diabetes Educator as part of the instructional team exclusively for the Outpatient DSME Program. Chairperson of the DSME Program Advisory Committee and liaison to the acute care hospital professional development staff for diabetes education.
Major Accountabilities/Critical Responsibilities:
- Responsible for maintaining the overall effective and efficient day to day operations of the Diabetes Management Department including staffing, scheduling, customer service, and policy and procedure.
- Selects, trains, and evaluates human resources to support the center including RN and RD diabetes educators, Nurse Practitioner diabetes educator, and reception personnel.
- In collaboration with the Executive Director of Ambulatory Services, creates and manages the operational and capital budgets, as well as budget variances and visit volume projections for the program.
- Develop, instruct, and evaluate individual/group DSME program sessions/plan in accordance with current National Standards for DSME & Support, American Diabetes Association diabetes care and treatment standards, age specific guidelines as well as religious and cultural practices.
- Chairperson of the DSME program multidisciplinary advisory committee in order to establish annual program planning and oversight, and the reporting of program clinical/behavioral outcome measurement.
- Facilitates the identification of evidence based CQI/performance improvement initiatives within the dept. and applies quality improvement process specific to SH to achieve a more effective program.
- Directs effort with the diabetes education team to identify and improve referral/marketing/community presentation strategies to achieve a more effective and accessible program.
- Responsible for monitoring key staff quality performance indicators in order to make concurrent adjustments to ensure projects within the diabetes education program stays on track. Uses clinical and behavioral outcome measures to gauge effectiveness of practice and DSME program.
- Maintains open communication with Diabetes and Endocrine Center team Physician(s), Nurse Practitioner(s), and other healthcare professionals to ensure excellent patient care, quality education, and understanding of department specific compliance with policies and procedures.
- Completes 15 hours of annual contact hours related to Diabetes Care and Nursing Education.
- Communicates with Executive Director Ambulatory Services with regards to team effectiveness and resource opportunities.
- Liaison with the property and facilities manager to make certain of facility and systems safety and facilitate prompt corrective actions.
- Ability to effectively communicate across the organization with hospital personnel and departments, outside vendors, as well as patients and community Physicians, in order to work as partners in completing challenging strategic growth opportunities.
- Participates in and supports performance improvement and risk management activities.
- Supports hospital-wide and unit based Nursing Shared Governance framework and initiatives.
- Participates in ongoing Community Health Outreach activities.
- Participates in peer review/feedback activities, self-appraisal, and goal setting to enable professional development.
- Stamford Hospital encourages nurses to actively seek ways to enhance professional knowledge and competence through participation in professional nursing organizations.
- Prepares patient for and administers treatment ensuring the safety and comfort of the patient and within the scope of CT State Regulations for Nursing Practice.
- Demonstrates professional work behavior by following Service Standards, Success Factors and Nursing Code of Ethics.
- Utilizes age specific competencies required include toddler, school age, pre-teen, teenager, young adulthood through geriatrics.
Qualifications/Requirements: BSN required. Current CT. Registered Nurse License. Minimum 5 years of recent clinical experience with evidence of diabetes related teaching and program management, and CDE (Certified Diabetes Educator) credentials required. Current certification in Insulin Pump and Sensor Training is required or to be obtained within 6 months of hire.
