This is a remote full-time position, allowing you to work from the comfort of your home while dedicated to facilitating the care of patients at our hospice center. Regular business hours will be observed, with flexibility to accommodate occasional evening and weekend shifts based on patient needs and caseloads. You will collaborate with a team of experienced rehabilitation professionals, utilizing advanced technologies and methodologies to support our patients effectively.

Your day-to-day tasks as a Licensed Clinical Social Worker will include:

• Conducting in-depth psychosocial assessments through one-on-one interviews, observations, and thorough reviews of medical records to identify emotional, social, and environmental factors that may be impacting patients’ recovery.

• Providing direct counseling and emotional support to patients and their families, helping them navigate adjustment issues stemming from disability, chronic illness, or trauma.

• Collaborating closely with the rehabilitation team to formulate personalized treatment plans that holistically address patients’ psychosocial needs alongside their physical rehabilitation goals.

• Empowering patients to access community resources, including support groups, vocational rehabilitation, and housing assistance, facilitating their successful reintegration into society.

• Participating actively in discharge planning processes, ensuring seamless transitions for patients returning home or transferring to alternative care settings.

You will primarily work with a diverse group of patients recovering from various conditions at our hospice center. Your expertise in addressing emotional resilience and coping mechanisms will be crucial in assisting individuals grappling with grief, trauma, and the profound lifestyle adjustments required post-rehabilitation. Our patient-focused approach is dedicated to providing them with the tools and support they require to successfully navigate their recovery journeys.

The ideal candidate will possess a Master’s degree in Social Work from an accredited institution, ensuring a strong foundation in clinical practices. Additionally, active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

Experience in a rehabilitation or medical setting is highly preferred, along with a robust understanding of adjustment issues, grief counseling, and coping mechanisms. Strong communication and interpersonal skills are essential for establishing rapport with patients, families, and team members alike.
