Bilingual Referral & Care Coordination Specialist
Community Health Association Inland Southern Region
Community Health Systems, Inc. (CHSI) in Bloomington, CA is seeking a Referral Coordinator to coordinate referrals with Providers and a multidisciplinary team, handling paperwork and notifying patients of referral status. You will ensure referrals are processed for follow-up treatment and scheduled accordingly in CHSI policy. The role requires a High School diploma or equivalent, with 1+ year of healthcare experience preferred.
