Speech-Language Pathologist (SLP) Job DescriptionI. Purpose and Overall DutiesThe Speech-Language Pathologist (SLP) evaluates, plans, and implements individualized speech and language therapy programs for patients in a long-term care or related setting. The role focuses on improving communication, cognition, and swallowing function to enhance patient safety and quality of life.II. Essential Job FunctionsA. Patient Evaluation and Treatment PlanningEvaluate patients’ speech, language, cognitive-communication, and swallowing abilities using standardized assessments and clinical judgmentDevelop individualized treatment plans based on patient needs, physician orders, and regulatory requirementsEstablish measurable goals and modify treatment plans as necessary based on patient progressB. Patient Care / InterventionImplement therapeutic interventions to improve speech, language, voice, fluency, cognition, and swallowing functionProvide dysphagia management, including assessment and treatment of swallowing disordersEducate patients and caregivers on communication strategies, swallowing precautions, and home programsRecommend diet modifications and adaptive strategies as appropriateC. Documentation / Compliance / OutcomesComplete all required clinical documentation accurately and timely in accordance with Medicare, regulatory, and company guidelinesMaintain appropriate records of patient evaluations, progress notes, and discharge summariesEnsure compliance with all federal, state, and payer regulationsMonitor patient outcomes and adjust interventions to support optimal resultsD. Collaboration / Interdisciplinary CoordinationCollaborate with interdisciplinary team members to coordinate patient careCommunicate effectively with physicians, nursing staff, dietitians, and families regarding patient status and recommendationsParticipate in care planning meetings and team conferencesE. Professional DevelopmentMaintain current licensure and clinical competenciesParticipate in continuing education and professional development activitiesSupport evidence-based practice and quality improvement initiativesIII. General Skills RequiredStrong clinical reasoning and decision-making skillsEffective communication (verbal and written)Interpersonal and relationship-building skillsAttention to detail and critical thinkingTime management and organizational skillsAbility to work collaboratively within an interdisciplinary teamIV. Minimum Experience, Education, and LicensureMaster’s Degree in Speech-Language PathologyCurrent state licensure as a Speech-Language PathologistCertificate of Clinical Competence (CCC-SLP) preferredOne (1) year of experience preferredV. Essential Physical DemandsAbility to sit, stand, and move between patient care areas throughout the workdayAbility to position or assist patients as needed for evaluation or treatment activitiesOccasional bending, reaching, or positioning of materials or equipmentAbility to demonstrate exercises or techniques related to speech and swallowing therapyAbility to use proper body mechanics in accordance with safety standardsVI. Work EnvironmentLong-term care or clinical settingAbility to travel to various locations where patients are located, including long-term care, short-term care, and private homes. This work will involve ability to enter and access buildings, work with patient in their existing space. Varying environmental conditions may be encountered while traveling.Indoor environment with occasional travel between patient areas or facilities as neededVII.
